Participation Waiver
I AGREE TO THE FOLLOWING RULES, TERMS, AND CONDITIONS:
No adults on the water slides with or without children.
No flips or jumping.
Feet first down the slides.
One child at a time down the slides.
No food or drinks in the slides.
No fighting or physical contact amongst children.
Children must wait to go down the slide until the previous child exits the pool.
Children must exit the pool to allow other children to use the slide.
No blocking the slide.
Adult supervision is required at all times.
Children must listen to the instructions and rules of the workers.
Waiver MUST be signed by customers prior to delivery and use of the water slides.
RELEASE OF LIABILITY:
I will ensure participants follow all verbal and written rules.
Participation poses a risk of injury and I, on behalf of myself and all participants who I am responsible for, freely accept all risks.
I, on behalf of myself and all participants, do not hold T.K.O. Splashbash responsible for injury.
